When the bewitched mother is on the bridge and drops the baby off the side you can see the dummy baby hit the side of the bridge and the dummies hand fly off of it and splash into the water beside the dummy's body.

What are the differences between the R-Rated and Unrated Version?

Like many other movies of the Dimension Extreme series, Argento's final chapter of his Mother-trilogy had been released in two versions in the US, too: in a censored R-rated version and in an uncensored original version as an unrated DVD. As this is an Italian movie, there are no extraordinary cuts, just some for censorship reasons. The R-rated version is even heavier cut than similar censored versions, due to some cuts of erotic material. A detailed comparison between both versions with pictures can be found here.
