When Sam leaves his apartment and Henry is waiting outside, as they walk downstairs the room they stop in front of to talk is number "21". At the end of the movie when the camera is panning up away from the scene of the accident, the number on top of the ambulance is "2121". They also live on the 21st floor; you can see the sign when Sam and Lyla discuss trust after he confronted her about not taking her medicine. This is in reference to Henry's 21st birthday, the recurring theme in the movie.

The original ending of the movie had Henry standing on top of the bridge (where he is last seen with Sam before the "truth" is revealed). He is shown as gray and spirit-like, looking down on the events that are happening on the bridge.

Goofs
All of the books in the "bookstore" that Dr. Foster visits as he is trying to find Henry have library identification labels on their spines, revealing that the scene was actually shot in a public library (the 58th Street branch of the New York Public library in Manhattan, as it turns out).

The answering machine in Henry's apartment has "Digital Answering Machine" printed on it yet when the new message is played it sounds like a cassette tape rewinding before the message is actually played. Digital answering machines do not have tapes to rewind so there should be no taped rewinding sound.
