Was originally set to be released through Cannon Films and to be directed by John Hough but the film was passed on to Menahem Golan's then-newly started studio 21st Century Film Corporation after Cannon filed for bankruptcy. Hough was replaced by Dwight H. Little.
The first version not to feature a falling chandelier. The chandelier scene was originally written in the first script by Gerry O'Hara. One of the reasons why this is the only Phantom film not to include that classic scene, is because Menahem Golan, CEO of 21st Century Films, who at the time had just resigned from Cannon Films in 1988 after that company's collapse, didn't have enough money to pay for an expensive chandelier to fall and then to redo the scene take after take. Duke Sandefur then was hired to rewrite the script without the scene.
Originally wanted to be rated X by the MPAA due to several scenes of graphic violence, after this happened the film was edited down very heavily and was re rated R.
When Christine visits her father's grave, we see several close-ups of the black wrought iron gate surrounding the cemetery. In these close-ups, you can plainly see that the wrought iron is actually wood that has been spray painted black.
When Desler is dragging Christine, he holds her right hand. The Inspector then shoots Desler and we see a close shot of him holding Christine's left hand to pull the ring off. The next shot is of Desler falling and letting go of Christine's right hand.
